Leaseholder request for disclosure of landlord’s identity - Section 1 of the Landlord and Tenant Act 1985
To:
[insert name of agent, or the person who demands rent, or the last person who received rent]

1. I am/we are the leaseholder(s) of the above property.
2. As the person who demands rent payable under the lease
OR
As the last person who received rent payable under the lease.
OR
As a person for the time being acting as agent for the landlord in relation to the lease.
3. I/we hereby request you to provide me/us with a written statement of the name and address of the landlord of the above property within the period of 21 days beginning with the day on which you receive this request. You are required to provide this, within the time limit given, under Section 1(1) of the Landlord and Tenant Act 1985.
A person who, without reasonable excuse, fails to comply with a request under Section 1(1) of the Landlord and Tenant Act 1985 commits a summary offence and is liable on conviction to pay a fine not exceeding level 4 on the standard scale.
The local housing authority has the power to bring a prosecution.
